Clyfton Unit: complex clinical care
Clyfton Unit supports adults with complex clinical needs that require ongoing, coordinated care from a skilled multidisciplinary team.
Most people living here have a primary neurological condition and multiple co-occurring health needs such as dysphagia, epilepsy, respiratory conditions, bladder and bowel needs, mobility impairments, or require clinical interventions like PEG feeding.
With a Registered Nurse on duty each shift, we provide a high level of nursing care, often supporting people who’ve transitioned from high-dependency hospital settings following brain or spinal injuries, or people in the advanced stages of conditions like Huntington’s or Parkinson’s.
Many individuals on Clyfton have limited mobility and require regular repositioning, skin integrity checks, and ongoing monitoring to reduce the risk of pressure ulcers and promote comfort, posture, and long-term health.
People here often have limited communication and engagement, so our team takes an innovative, sensory-led approach to enhance wellbeing and quality of life.
This care goes beyond what’s typically provided in standard residential or nursing care settings, and offers a more holistic, personalised approach than hospital environments - combining clinical expertise with meaningful engagement and enhanced quality of life.
Where there’s rehabilitation potential, individuals can transition to Bridgeway Unit within the same home to build greater independence.
For those with progressive conditions, we support person-led advance care planning, ensuring their wishes and preferences for end-of-life care are known and respected.
Bridgeway Unit: clinical care focusing on recovery and independence
Bridgeway Unit supports adults with complex clinical needs who are medically stable but still require nursing input.
Care focuses on maintaining health, promoting independence, and enhancing community and social engagement.
People on Bridgeway tend to have higher levels of mobility and communication than those on Clyfton Unit, and are more involved in self-care and daily routines.
The unit has an adapted therapy kitchen, two apartments, and a private garden, offering a supportive, homely environment that supports progression and promotes independence.
The home also has a beauty therapy room and a cinema room, helping people build confidence and skills in using these services before accessing them in the community.
Whether someone is continuing their recovery journey or living well with a long-term condition, we provide personalised care that balances clinical support with a strong focus on quality of life.
Mayflower Unit: behaviours of concern with complex risk
Mayflower Unit supports adults who display frequent, high-risk and/or unpredictable behaviours that pose a significant risk to themselves or others. These behaviours are often linked to neurological conditions or mental health diagnoses, including acquired brain injuries, dementia, and Huntington’s disease.
Led by a Registered Mental Health Nurse, the team provides structured, evidence-based behavioural support.
This includes functional behaviour assessments, individual support plans, proactive strategies to minimise escalation, skill building, and safe reactive strategies - all underpinned by our Exemplar Positive Behaviour Support approach.
Many people move to Mayflower following the breakdown of previous placements - often because other providers don’t have the expertise, skills, or staffing levels to deliver this level of care safely and consistently.
We provide enhanced support with high staffing levels and observation levels based on risk, including 1:1 or 2:1 support when needed.
Over time, we often see behaviours and observation reduce as people settle into a structured and person-centred environment.
For those with degenerative conditions, we support transitions to other units in the home as clinical needs increase, ensuring continuity of care in a familiar environment.
Pilgrim Unit: proactive care for lower-risk behaviours
Pilgrim Unit provides a safe, structured environment for people who display behaviours of concern at a lower level of risk.
People here may have neurological or mental health conditions but don’t require high-level interventions or constant observation.
This unit is ideal for those who are vulnerable or would be unsettled by exposure to more intense behaviours.
Our approach centres around proactive support, communication strategies, routine, and meaningful occupation - all within a vibrant, social setting.
Pilgrim offers a balance of safety, structure, and community connection, where people enjoy being part of daily life both in the home and beyond.
