About our care

Our community-based homes provide person-centred care and rehabilitation that focuses on maximising independence, building everyday living skills and empowering people to live as fulfilled lives as possible.

We support people on their journey from being in hospital or living in a secure setting to community-based living, as well as offering longer term support for people living with degenerative or life-limiting illnesses.

OneCare services

Alongside many Exemplar Health Care homes, we have OneCare services which support people to live in a fully adapted house or flat, that promotes independence, whilst accessing 24/7 support from a dedicated Nursing Team. 

OneCare services

Our OneCare service is ideal for anyone who wants to move forward with their life and live more independently, with the support of a specialist nursing team. People can practice their living skills with planning meals, shopping, cooking and housework.

Caring for complex conditions

Exemplar Health Care is a leading provider of specialist nursing care for adults living with a range of complex and high acuity needs, arising from neuro-disability, brain injury, spinal injury, stroke, mental ill-health, physical disability, learning disability and/or autism and complex dementia.

Before my daughter came to Fairburn Mews she was emotionally distressed - but she is in a much better place now. The staff at Fairburn Mews are very supportive to her and myself, and have gone above their duty - they are amazing.

Service user engagement

We listen to our service users and involve them in decisions that impact their life and the running of our homes. Residents can become a Service User Ambassador and take part in our Service User Council, as well as having regular opportunities to get involved and feedback.

I am one of the Service User Ambassadors at Scotia Heights. It’s fantastic that I can get involved and help to influence things. I have already made a difference in the home by putting information onto the service user boards, reminding everyone about our service user of the day award and going to the morning meetings with the Activities Team to share my ideas with them.
