Our therapy goals

Through our therapy services, we aim to:

  • work as part of the in-house multi-disciplinary team to provide holistic care tailored to each person’s needs

  • promote independence by building daily living skills

  • support community and social engagement to enhance overall wellbeing

  • collaborate with care team and external professionals to ensure coordinated care and shared goals

  • integrate rehabilitation into daily life to foster an environment that encourages and supports independence in all aspects of care.

Physiotherapy interventions

Our physiotherapy services focus on building mobility and maximising independence through:

  • aids and equipment: assessing and providing aids, adaptations, specialist seating and wheelchairs to enhance daily living and mobility

  • assessments: creating personalised treatment plans with assessments for equipment needs, falls risk, mobility and safe handling

  • falls prevention: developing personalised strategies to prevent falls, promote self-management and improve physical function

  • group exercise classes: offering classes to boost strength, flexibility, endurance and posture, while reducing pain and stiffness

  • mobility support: recommending exercises and techniques to improve balance, coordination and strength

  • spasticity management: using positioning, splinting, manual therapy and rehabilitation to manage spasticity and muscle tone, maintain function and ensure comfort.

Occupational therapy interventions

Our occupational therapy services identify people's strengths and difficulties, and develop person-centred treatment plans to help them work towards their goals, including:

  • daily living skills: helping with essential self-care activities like cooking, managing finances, personal care and shopping

  • meaningful occupation: collaborating with the wider team to support activities that provide a sense of identity, connection and accomplishment

  • vocational support: assisting with educational, employment or volunteering opportunities

  • discharge planning: ensuring a smooth transition back to the community by coordinating with family and multi-disciplinary teams to prevent any issues after discharge.

Meet Helen Wilson, Lead Physiotherapist

Helen Wilson is our Lead Physiotherapist with extensive experience in treating people with complex needs.

Her career spans private practice, community settings and care homes, with a focus on managing long-term and neurological conditions.

Helen’s passion lies in delivering personalised rehabilitation that enhances independence and quality of life while supporting both individuals and their families in managing their conditions.

She’s been with Exemplar Health Care for over three years, where she initially made a significant impact at Tyne Grange in Newcastle upon Tyne.

There, she helped people achieve their rehabilitation goals and played a key role in the home’s ‘Outstanding’ CQC rating by developing a robust therapy structure and team.

Now, Helen’s focused on advancing therapy strategies and upskilling our therapists, driven by her commitment to quality and clinical excellence.

Therapy and behaviour support

Our therapists collaborate with the behaviour support and nursing teams to align rehabilitation goals with individual abilities and limitations.
