To request a reference, please contact the care home/team that the candidate worked in directly.

If that's not possible, contact our General Enquiries Team (above) with the following information:

• name of colleague

• the name or address of the care home/team they previously worked at with Exemplar Health Care.

Failure to provide this information may result in delays.
