At Exemplar Health Care, we understand that a person’s mental health is a journey, and that everyone’s experience of mental health is different. Our holistic approach to care involves working with individuals, their loved ones and professionals to design a care plan that’s tailored to their needs and goals, including re-integration into the wider community.

Our specialist, Nurse-led teams support people living with a range of complex mental health needs, including anxiety, depression, personality disorder, bi-polar affective disorder and schizophrenia.

We support people on their journey from being in hospital or living in a secure setting to community-based living, as well as offering longer term support for people living with life-limiting conditions.

Our homes care for some of the most complex and acute individuals, who would often be in a hospital setting if not placed with us, enabling them to stay in the community and reducing hospital admission.

High staffing levels to support person-centred outcomes

Our high staffing levels of Registered Nurses and trained Health Care Assistants are determined by each person’s individual needs, which can include constant observation, either 2:1 or 1:1.

This enables us to work at their own pace and build trust, and provide support with all aspects of their life, leading to improved person-centred and longer-term outcomes for those living with complex mental health needs.

Our community-based care homes and OneCare services provide person-centred care and rehabilitation that focuses on maximising independence, building everyday living skills and empowering people to live as fulfilled lives as possible.

We understand that the right home, with the right support, is an essential part of overcoming mental health challenges, which is why our care homes provide a welcoming and supportive environment where people can feel at home, continue to do the things that they enjoy, and remain closer to family, friends and loved ones.

Alongside many of our homes we have OneCare services which provide specialist nursing care in a single or dual occupancy domestic environment. They’re ideal for those whose needs require that they live alone or in a low stimulus environment, or who’d like to build their independence as part of their journey to lower acuity settings or community-based living. 

Holistic care from an in-house Nurse-led team

Our homes are led by an experienced Nurse-led management team, and supported by a trained in-house team of Registered Nurses and Health Care Assistants.

Each of our homes has a Life Skills/Activities Team to support with holistic and therapeutic treatments, and many of them have in-house Physiotherapists and Occupational Therapists. Everyone who lives in our homes also has the support of Exemplar Health Care’s clinical experts, including Behaviour Support Specialists and Consultant Psychiatrist at times of increased distressed or crisis.

This enables people to access holistic care and a range of services in a community-based home that’s closer to their friends and family.

We also work closely with community teams and other professionals, where required, to ensure that people receive the required support to meet all of their needs.

  • Life skills – our Life Skills Teams support people to maximise their independence, build their everyday living skills and take part in meaningful activities that enable them to live a fulfilling life.
     
  • Occupational therapy – our Occupational Therapists help people to overcome the effects of their mental health condition, by considering their physical, psychological, social and environmental needs.
     
  • Behaviour support – our dedicated Behaviour Support Team work with individuals and care teams to understand the reasons for people’s behaviour, and implement strategies to reduce the frequency and duration of incidents of behaviours of concern, in order to support people to re-integrate with society. The team is supported by our Consultant Psychiatrist.

A personalised approach

Our approach is personalised to the individuals that we support – we work with them, their loved ones and professionals to design a care plan that’s tailored to their needs and goals, whether that’s moving to a lower acuity service or community living, or maximizing their potential in one of our care homes.

Our collaborative approach encourages self-development, self-confidence and improved emotional wellbeing, to support people on their pathway to rehabilitation and recovery.

We support and empower people to build essential community links and strategies that support re-integration into the wider community. 

Behaviour support for people living with complex mental health needs

People living with complex mental health needs can also display behaviours of concern.

Many of the people we support have been deemed to have behaviours that are considered ‘too complex’ for mainstream nursing and care services.

Our approach to behaviour support focuses on how we can prevent the need for restrictive practices and reduce the frequency and intensity of behaviours of concern.

All of our care and nursing colleagues complete our Exemplar Positive Behaviour Support (EPBS) training that’s been certificated by Bild ACT, and are supported by our central team of Behaviour Support Specialists through person-centred interventions, fortnightly clinics and regular advice forums.
