Your role will include providing specialist assessments, clinical diagnoses and interventions to support people to maintain optimal muscle function, limit/delay deterioration and regain movement and mobility where possible.
You’ll work with a wider care team and clinical team to create and implement person-centred care plans to support people to achieve their goals and evaluate their outcomes.

You’ll support people with a range of health conditions arising from brain injuries, dementia, neuro disabilities such as Huntington’s disease, physical disabilities and spinal injuries.

What are the day to day activities of a Physiotherapist at Exemplar Health Care?
As a Physiotherapist with Exemplar Health Care, you’ll support the service users in your home to live a fulfilling life by providing the highest standards of care.
No two days will be the same – our service users are individuals and they deserve to be treated as such.
As a general guide, your days could involve the following:
being responsible and accountable for a clinical caseload in your home
working with service users, their loved ones and other professionals to develop and deliver person-centred care plans for physiotherapy interventions
carrying out skilled assessments
ensuring that people receive the right therapy interventions and treatments
advising and mentoring colleagues that may need your guidance.

If your application is successful, you’ll do a welcome induction which covers mandatory and statutory training including moving and handling, safeguarding and person-centred care. After that, you’ll continue your induction in your home with the support of a network of other therapists.
