Our OneCare services provide specialist nursing care for adults living with complex needs, in a single or dual occupancy domestic environment, ideal for those whose needs require that they live alone.

Our OneCare services provide specialist nursing care for adults living with complex needs, in a single or dual occupancy domestic environment, ideal for those whose needs require that they live alone.
Our OneCare services are a mix of single and dual occupancy flats and houses that are located in close proximity to our specialist nursing homes, often just over the road or down the garden path!
This close proximity means that people can be as independent from, or integrated with, the main nursing home as they wish or as is required, and always have the support of an experienced multi-disciplinary team of professionals just a few steps away.
If needed, people can also access short-term support from the nearby home to avoid a re-admission to hospital.

As well as providing a home for those who benefit from living alone, some people live in our OneCare services as part of their rehabilitation journey to lower acuity care settings, or as part of their journey into our specialist care homes, such as those in the early stages of Huntington’s disease.
Our OneCare services offer a bespoke package of care that’s developed to meet people’s complex and individual needs.
As well as providing a home for those who benefit from living alone, some people live in our OneCare services as part of their rehabilitation journey to lower acuity care settings. Others live in our OneCare services as part of their journey into our specialist care homes, such as those in the early stages of Huntington’s disease.
OneCare offers care, nursing and accommodation costs in one weekly fee, which enables people to progress on their journey at their own pace, without the need to apply for other benefits or set up utilities, and allows a straightforward reassessment and transfer between our nursing homes and OneCare services as required by the individual.
Paul lives in the OneCare service, which is over the road from our Dearnevale care home in Grimethorpe, Barnsley.
Following a road traffic accident which resulted in an acquired brain injury, he originally moved into the main care home in 2009. When Paul first moved to the home, he required a wheelchair to aid his mobility, and he displayed behaviours of concern.
The team worked with him to promote his independence and support his rehabilitation, and a few years later, Paul moved into his own flat in the OneCare service over the road.
There, he’s supported to build his everyday living skills whilst still receiving specialist nursing care from a trained team of Health Care Assistants and Nurses. He enjoys looking after his garden, going swimming and playing on the local disability football team.
Paul’s long term goal is to get his own place near his family or in Sheffield where he used to live.
